I am trying to develop a web-based survey instrument that would allow educators to assess several outcomes related to their education programs and thus provide data for improving the delivery of those programs.  Is anyone interested?  Please reply directly to me by email.

I coordinate the childbirth and parenting education for a hospital with nearly 10,000 births a year.  I tried using a Survey Monkey questionnaire for two years.  It was moderately informative.  As usual, the verbatims were the most useful.
