Applicants should refer to these guidelines while preparing their online grant applications. Note: Applicants are encouraged to use this outline to prepare online grant submissions in a word processing program and then paste into the online grant application when all responses are complete.
Before completing an application, applicants must read the program and eligibility information for the Birth Network Mini-Grants or Community-Based Outreach Mini-Grants.
Birth Network and Community-Based Outreach Mini-Grant Applications must include the following:
- Applicant Contact Information
- Name of organization
- Contact person for grant application inquiries
- Mailing address, phone number and e-mail address
- Official recipient of grant funds, if awarded (check payee)
- Lamaze member name & ID (community-based outreach applicants only)
- Geographic area served
- Proposal Summary: Describe the project you are proposing, indicating how it will satisfy the goals of the Birth Network Mini-Grant Initiative or Community-Based Outreach Mini-Grant Initiative. (Limit response to 100 words.)
- Project Director and Collaborators: Identify the project director and major project collaborators. Describe their background, interest in normal birth and involvement in the project. If a collaborator also offers a financial contribution, it should be noted as well. (Limit response to 250 words.)
- Project Beneficiaries: Who will benefit from your project? Describe the approximate number (if known) and any key characteristics of the population your project will serve. (Limit response to 100 words.)
- Objectives, Activities, and Timeline: Identify the objective(s), or desired outcome/impact, of the proposed project. For each objective (desired outcome/impact), describe the proposed activities that will help you achieve that objective, and timeline for implementation (when the activities will start and end). Grant funds awarded in December must be used within the following calendar year. (Limit response to 500 words.)
- Evaluation Measures: For each objective, how will you evaluate success? Be sure to use evaluation indicators that you are able to measure with your organization’s resources. (Limit response to 250 words.) If your proposal is accepted, you will be asked to submit a Final Report by December 31 of your project year, to include a project summary with the results of your project evaluation and a detailed review of actual expenditures of the
grant funds.
Note: Prior grant awardees are required to submit an
interim project summary narrative prior to submitting an application for an additional grant.
- Budget: If your proposal is accepted, your final report must include actual expenditures of the grant funds. Small deviations in budgeted expenditures that stay true to the intent of the project are acceptable. Project directors should maintain appropriate files, which may be audited, containing receipts, invoices, and cancelled checks.
- Total budget for proposed project
- Amount requested from Lamaze
- Other sources of project support, which may include the value of in-kind (donated) contributions, but may not include a value for volunteer time
- Project budget detail, including justification for all requested items by category of expense. Example expense categories include: consultant fees/purchased services; expendable supplies; equipment; and other direct costs.
- Web Site, Promotional Brochures or Supplemental Material: Please provide a Web site address for your organization or birth network, if one is available. Please also list links to or names of promotional brochures or other materials that are available upon request.

- Total birth network annual budget
- List of officers and a general description of the membership
- Mission and governance (Please describe the birth network's mission, organization structure and decision making process.)
